Poor knee positioning can delay your recovery.

Knee recovery is hard work. Even with excellent physical therapy, progress can stall. Most setbacks happen when your leg is in the wrong position — leading to swelling, stiffness, and soreness.

It may seem small, but poor positioning can make your recovery longer, harder, and more painful than necessary. The right positioner changes that from day one.

Whether preparing for surgery or managing everyday knee pain, recovery is smoother when your leg is supported properly. Watch to see how our positioners help you heal faster and more comfortably.

The clinical foundation

The Positioner is the tool that makes the Smooth Knee Protocol work.

The Smooth Knee Protocol prioritizes edema control and early ROM in the critical first 10–14 days after surgery. The Positioner holds the knee in terminal extension and keeps the leg elevated — making both possible from the moment surgery ends.

★★★★★
"I had a total knee replacement done 5 years ago and the postoperative course was very difficult. Last year I had my other knee replaced. This one was so much easier, thanks to the only thing that changed — the use of the LRU Positioner."
— Nancy, age 60
★★★★★
"I had my first knee replaced and used the CPM machine. On my second knee, I used the LRU Positioner. What a difference! I could control my swelling, and my knee was completely straight. I can walk without a limp."
— Dorothy, age 64
★★★★★
"When I had my 2nd ACL surgery after the first one failed, it was tremendously more easy. Your approach to exercises and swelling control with the LRU Positioner made the difference. I'm now back living my life with a stable knee."
— Joe, age 35

"I don't know why everyone doesn't rehabilitate their knees the way you do. I had both of my knees replaced and I have much better range of motion and walk much better than anyone else I know that has had their knee replaced."

Frequently Asked Questions

How do I use the Positioner?
Place the positioner anywhere you will be sitting or lying — couch, bed, recliner, or floor. Place the leg in the positioner with the bottom of the foot lined up with the end of the positioner. Use it while sitting or lying, and especially while sleeping. Use it as much as possible — even after exercise — to speed recovery, relieve pain, and improve rehab results.
How long should I keep my leg in the Positioner?
Use the Positioner as much as possible during the first 10–14 days — this is the critical window for swelling control. The Smooth Knee Protocol recommends limiting time with the knee in a dependent position to 10 minutes at a time. Between exercises, return to the Positioner immediately.
Should I sleep with the Positioner?
Yes — especially in the first 10–14 days. Using the Positioner while sleeping helps control overnight swelling and keeps the knee in the proper position through the night. Many patients find it more comfortable than sleeping without it.
Should my heel hang over the edge of the Positioner?
Yes. The heel should hang just off the end of the Positioner. This offloads pressure from the heel, reducing the risk of pressure ulcers and allowing the knee to fully extend.
Can I use the Positioner in a recliner?
Yes — the Positioner works on a bed, couch, recliner, or floor. Many patients find the recliner the most comfortable option for daytime use as it combines full leg elevation with an easy position for getting up and down.
What if the Positioner is too long?
Position it so the end aligns with the bottom of your foot. The length is designed to support the full length of your leg. If needed, the positioner can be placed on a pillow or wedge to adjust overall height.

The story behind the Smooth Knee Protocol

Physical therapist Scott Johnson and orthopedic surgeon Dr. Matthew Lawless share more than two decades of working side by side — the patterns they saw across thousands of recoveries, why swelling control and early range of motion are the real keys to a successful knee surgery, and how it led them to develop the Smooth Knee Protocol and the ElevateX Positioner.
